Overview
Starting Rate: $24.50
Schedule: Monday through Friday 9am-5pm with flexibility to meet the needs of the program
To coordinate, provide and supervise high quality supports to the individuals and providers served by Shared Living program.
Minimum Education Required Bachelor's Degree
Responsibilities Responsibilities include, but are not limited to:
Implementation of all aspects of the program.
To act as program liaison to local DDS Area Office, Survey & Certification, and other Department personnel as delineated by Program Director.
Act as liaison with other agencies involved in providing services to the people we support such as day programs.
Ensure that the program meets and complies with all applicable DDS/Medicaid regulations: overseeing compliance of providers with mandated trainings.
All responsibilities as identified in DDS regulations re "placement services” Process participant referrals and aspects of the coordination of the admission process.
Collaborate with the Development Coordinator and participate in recruitment, selection, and evaluation of shared living families.
Interview families and facilitate the matching and transition of individuals into homes.
Develop positive relationships with individuals, families and monitor/assess placements minimally on a monthly basis. Train staff and families in all facets of their responsibilities.
Requirements
BS/BA in related field with 4 years of experience working with individuals with developmental disabilities.
Ability to communicate effectively verbally and in writing and able to use good judgment.
Must be able to perform each essential duty satisfactorily.
Sensitivity to the needs of the individuals we support.
High energy level, superior interpersonal skills and ability to function in a team atmosphere.
Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
