Position Description
The Transition Services Coordinator reports to the Chief Program Officer and serves on the STEMWorks Team, working with Career Launch program. The two main job expectations for this position are securing employer partners who will interview Career Launch graduates for employment and supporting youth with enrolling into post-secondary or vocational programs and/or employment. This position will coordinate with healthcare, manufacturing, IT, and transportation/logistics industries.
PRIMARY RESPONSIBILITIES
- Ensures orientation and career assessments are completed, and individualized career plans are created in coordination with other program staff.
- Implement lessons for resume writing, interviewing skills, networking, and professional communication and tweaks lessons for continuous improvement.
- Assists participants with completing applications for post-secondary school, certification programs and/or employment.
- Maintain relationships with existing employer partners as well as secure new employer partners. Works with employers to ensure the timely and successful transition of program graduates.
- Coordinates with program manager and partners to schedule offsite visits and in person presentations to help inform participants decision making when choosing career paths.
- Conducts the required post-program proactive follow-up of graduates and enters and maintains program graduate information in the applicant tracking system.
- Collects and reports data into the grant portals to keep in compliance with funder requirements.
- Build relationships through alumni networking for current and former participants.
- Provides participants with guidance in their career path, including but not limited to supporting certification application and completion, job readiness, and ongoing workplace skills training.
- Provides participants with job assistance to help secure employment upon completion of the Career Launch program.
- Develops a mentorship program that supports participants through all the stages of job readiness to acquire and maintain employment.
- Collaborates with program manager, case manager, and facilitators to ensure the development of job readiness skills for participants.
- Other duties as assigned.
Requirements
Workforce development
o Experience with workforce development programs helping young adults prepare for the workplace.
o Connection with various industries including healthcare, manufacturing, transportation and logistics.
Technology Skills
o Advanced level proficiency with Microsoft Office Suite including Excel, Word, Outlook and PowerPoint; Ability to learn and adapt to new technologies quickly.
Detail/Accuracy
o Demonstrate accuracy and thoroughness; Monitor your own work to ensure quality, organization, and timely execution; Maintain strong attention to detail even while multitasking.
Communication
o Clearly and persuasively communicate, listen, and seek clarification; Ability to lead meetings, provide support to Board committees; Demonstrate excellent written and verbal communication in all activities.

o Demonstrate ability to work independently and as part of a team; Exhibit strong critical thinking skills and sound judgment including considering the safety and wellbeing of youth served; Prioritization and time management skills.

Work Schedule/Location- Full-time, 37.5 hours/5 days a week. There will be periodic evening and Saturday work. This position works out of 1010 W Madison St. in Maywood as well as 1132 S. Wabash Ave, in the South Loop of Chicago.
